Behavior: During the course of the visit, client displayed excellent leadership skills by listening, paying attention, and contributing to discussion without feeling insecure or inadequate within her home environment. Client displayed appropriate behaviors while listening to QMHP share information regarding her disorder. Client also shares her daily experiences, in order to reflect and debrief during session. Client expressed positive thoughts as she interacted well in the home with her family, who were all rooting for her and in support of her treatment. 2. Behavior: Client appeared to be an energetic mood while helping mom take care of duties around the house. Client appeared way happier and way more focused during todayâs session time. Client was praised for being responsible and not giving authority a hard time when prompted to do different tasks. Client was able to transition from one activity to the next without feeling or needing to stop the session, decreasing all attention seeking behaviors and participating fully in her therapy. 1. Intervention: QMHP helped client state, demonstrate, and identify how to communicate her feelings appropriately. QMHP utilized active listening skills as client spoke of her current mood and feelings, in regard to her mental disorder. QMHP processed and reiterated with client about her role in the home with watching over her little sister and taking care of her dogs when her mom is busy working or doing clientâs hair. QMHP assisted the client with understanding the expectations that have been established in the home and community. 2. Intervention: QMHP went over her objectives with client to help her understand the importance of following them, so that she can be more self-aware. QMHP processed with client thinking questions to get her to think about things she normally avoids processing. QMHP will teach client necessary coping skills to decrease impulsive behaviors and careless decisions. QMHP will help client develop healthy self-esteem by reducing level of anxiety and emotional reactions related to home and other independent functioning. 1. Response: Client understood she was responsible for protecting her sister and the dogs, in order to be taken care of financially by her mom who works overtime shifts to provide for her. Client verbalized that sheâs able to focus better when it is quieter and more peaceful in the home than when itâs loud and out of control. This allowed client to concentrate in the session. Client was able to focus on one thing at a time instead of being all over the place and getting distracted by external stimuli. Mom noticed the change in her behaviors instantly. 2. Response: Client understood the rules in the home and the impact she makes on her family when she is aligned and incoherence with her objectives. Client understood the expectations that were established in the home and in the community, and how to conduct herself in each setting. Client was able to answer the thinking questions with assistance from QMHP. Client communicated that she feels she will be more productive in the home, due to consistent support in the home and learning to control her attitude.
